⦁ Your art may be any medium – it just has to be submitted as a PDF in this form
⦁ Artist must include name, phone number, and social media handles in their submission

⦁We are looking for 2 designs
⦁One design will be used for a T-shirt and the other for a Tank Top.
⦁ HEAL Utah staff will pick the top works of art and then the public will vote on the top choices
⦁ The final 2 winners will receive a 200 cash prize, along with recognition
